Application Procedures

You need Adobe Acrobat Reader to view and print forms.

Thank you for applying with the Lakewood School District. Our application selection procedures are as follows:

1. Complete a district Classified Application Packet which includes:

a) District application
b) I-9 form (a copy of your driver's license and social security card are required and a copy will need to be made for our records).
c) Disclosure form
d) Sexual Misconduct Form. (Complete one form for every school district you have been previously employed)

2. Complete and sign the W-4 and retirement forms. A copy of your driver's license and social security card is required. (your pay cannot be processed without this information on file)

3. Washington State Law requires that any person hired by a school district must be fingerprinted for a state and a national background check. Receipt of that clearance is a requirement for employment. The cost of this background check is $55.00 payable to the Washington State Patrol in the form of a cashier's check or money order. The cost is to be paid by the applicant.

The district will supply the fingerprint card. After you have had your fingerprints taken, you will need to bring the card, along with the $55.00 cashier check to the Human Resources Office. The district will mail the fingerprint card and cashier check or money order to the Washington State Patrol for processing.

4. Please attach, to the application, a letter of interest that lists the position(s) you are applying for and the job posting number.

5. All contracts are contingent upon satisfactory completion of a background investigation, which will include sexual misconduct allegations and convictions.

If you have any questions, please call Dawn Arrington, Human Resources Officer at 360-652-4501, ext. 1011
