Out Of District Registration
If you live outside the Lakewood School District please come to the district office located at 17110-16th Dr NE, Marysville, WA for the necessary forms. For more information, please call 360-652-4500.

In District Registration
Registration is done at the school where your child will be entering. A link for elementary boundaries is available as a pdf or jpg for magnified viewing.

During the summer please check with the district office at 360-652-4500, if you have any questions. Documentation that you may need will depend on the grade your child is entering. This may include but is not limited to the following:

1. Your child's immunization record. Please note the Washington School Immunization Law: For the protection of every student, a new Washington school immunization law requires that each child be properly immunized in order to register for school. This must be done on or before the first day of school. School attendance will be denied to all students not in compliance with immunization law. For information, please call the district nurse (360)652-4517 or visit the immunization Web site.

2. Your child's birth certificate. (This may include hospital record or baptismal record)

3. Be sure to have all addresses and phone numbers. You will need two emergency contacts in addition to your home and work contact numbers. These emergency contacts will be used in cases of illness or other emergency when you cannot be reached. They should be people who can come and get your child if he or she is ill.

4. Proof of residency. This can be a utility bill or tax statement with your name and current address listed on it.

5. Report card or Transcript, Attendance Records, and Discipline Records.

Visit our food service page for Free/Reduced lunch applications. Washington state recently passed legislation to allow any student who is approved by the school district to receive reduced price meals at school to automatically receive free breakfast at school everyday in the 2006-2007 school year. Students who are approved for free meals at school will continue to receive all meals at school at no charge.
