Thank you for choosing to participate in Lakewood High School athletic programs. After viewing and filling out the paperwork, these are the sheets that need to be turned in to the athletic office. The athletic clearance table will be set-up to take paperwork on August 16th and 17th at 8:30 am through 10:30 am, and 12:30 pm through 2:30 pm.
