Thank you for choosing to participate in Lakewood High School athletic programs.
After viewing and filling out the paperwork, these are the sheets that need to be turned in to the athletic office.

1. Registration sheet - all information filled out, signed by Athlete and Parent.
2. Emergency Card
3. Physical forms
4. Signed Co-curricular
5. Safety Guideline that relates to your sport signed.
6. Receipt of paid participation fee and ASB card.

Athletic clearance table will be set-up to take paperwork on August 16th and 17th, 8:30 am thru 10:30 am and 12:30 pm thru 2:30 pm.
