Registering for
Kindergarten:
All children entering
Kindergarten must be five years of age by August 31, 2008 for
the 2008-2009 school year. They also must have completed the three
shot Hepatitis B vaccine series and received the chickenpox vaccine
(if given before 1st birthday another dose is needed).
To register your child
for school at Lakewood Elementary, you will need the following:
1. Your child's immunization
record. Please note the Washington School Immunization Law: For
the protection of every student, a new Washington school immunization
law requires that each child be properly immunized in order to
register for school. This must be done on or before the first
day of school. School attendance will be denied to all students
not in compliance with immunization law. For information, please
call the school nurse (360) 652-4520.
2. Your child's birth
certificate if you are registering the child for kindergarten.
Also acceptable as proof of your child's birthdate are the commemorative
certificate from the hospital or a baptismal certificate, as long
as they have the child's name and date of birth.
3. Be sure to have
all addresses and phone numbers. You will need two emergency contacts
in addition to your home and work contact numbers. These emergency
contacts will be used in cases of illness or other emergency when
you cannot be reached. They should be people who can come and
get your child if he or she is ill.
4. Proof of residency
in Lakewood School District. This could include a PUD or other
utility bill, or any other various forms of proof of residency.
