Nutrition influences a child's development, health status, well-being and potential for learning. As partners in education, it is our mission to nourish students and the school community by preparing and serving attractive, delicious, healthy meals.
The Child Nutrition Eligibility and Education Benefit form is now available when you log in to Family Access under Food Services. In order to receive meals at a reduced price, students at Lakewood Middle School and Lakewood High School must have a current Child Nutrition Eligibility & Education Benefit (CNEEB) application approved on file at their school. Only one application per family is required.
Elementary students will all receive free meals this year as part of the district's qualification for the Community Eligibility Provision (CEP), a program that allows all students to receive one free breakfast and lunch each day.
Elementary families are still asked to complete the CNEEB form so that our elementary schools remain eligible for free meals and other funding that is dependent on the completion of these forms. By completing the CNEEB form, you help ensure your school receives important resources that help all students.

To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA Program Discrimination Complaint Form which can be obtained online at: https://www.usda.gov/sites/default/files/documents/ad-3027.pdf, from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
